In 2016, the program recorded a deficit of over six trillion IDR, which grew to 32 trillion IDR within three years. In response, the government increased premium rates by 80 to 100 percent, which led to concerns about affordability among lower-income citizens. Another challenge is hospital overcrowding, as many public hospitals struggle to accommodate the high number of BPJS Kesehatan patients. Some hospitals have also reported delays in reimbursement, which affects the quality of care provided.
To address these issues, BPJS Kesehatan has implemented several reforms, including digitizing healthcare access, which allows participants to register, check their insurance status, and schedule appointments online. The agency has also expanded partnerships with private hospitals to increase accessibility and introduced telemedicine services, enabling remote medical consultations to reduce hospital congestion and improve efficiency.
Despite these challenges, BPJS Kesehatan remains a cornerstone of Indonesia's healthcare system, striving to provide sustainable and equitable access to medical services. As the program continues to evolve, further reforms and technological advancements will be essential in ensuring its long-term success and effectiveness in delivering quality healthcare to the nation's population.
